American Traveler is hiring a Registered Nurse - Hospice for a home hospice and palliative care role requiring 2+ years of home hospice RN experience and a valid NJ RN license.

Details

• Home hospice and palliative care setting within a hospital-affiliated homecare program
• AM shift, 8a-4p, 5 days per week, 37.5 hours weekly
• Every other weekend required with comparable days off during the week
• Floating to different counties as needed based on demand
• May float to a nearby sister hospital
• No on-call responsibilities
• Uses Epic EMR for charting

Requirements

• Active New Jersey RN license or compact license required
• BLS certification required
• Valid New Jersey driver's license required
• Minimum 2 years of RN experience in a home hospice environment required, 3+ years preferred
• Associate's (ASN) degree or diploma with NJ RN license required
• Experience using an EMR required
• Strong critical thinking and communication skills required

Additional Information

• Perform patient assessments to determine hospice appropriateness, learning needs, and progress toward goals
• Develop, implement, and revise individualized plans of care for patients and families
• Complete admission, discharge, and re-visit documentation and data transfers within required timeframes
• Provide age-appropriate patient and family education and document teaching tools used
• Manage caseload with attention to visit productivity standards and insurance reimbursement guidelines
• Supervise home health aides and ensure appropriate utilization of services
• Participate in interdisciplinary team (IDT) meetings every two weeks and communicate patient progress
• Maintain a safe environment through infection control practices and medication reconciliation
• Attend monthly staff meetings and participate in performance improvement activities
• Provide orientation and mentorship to new employees as needed
• Local candidates accepted
