American Traveler is hiring a Home Health RN Float requiring 1 year of certified home care and case management experience along with an active MA RN license.

Details

• Home Health setting providing care to patients across the North Region including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End
• 13-week assignment with a schedule of 08:00-16:30
• Weekend rotations required once every 6 weeks
• Ability to travel to patient homes/cases as assigned
• Uses Epic for charting
• Uses UKG/Kronos for timekeeping

Requirements

• Active MA RN license required (no conditional or pending licenses accepted)
• Both NURSYS and Board of Nursing verification required for licensure
• AHA-issued certifications required
• 1 year of Certified Home Care experience required
• 1 year of Case Management experience required
• Flu vaccine required with no religious or medical exemptions allowed
• 2 professional references required for consideration

Additional Information

• Responsibilities include assessing patient conditions, implementing individualized care plans, and providing ongoing education and support to patients and families
• Perform admission processes and case management duties in accordance with organizational policies and standards of practice
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Deliver care aligned with organizational philosophy and community health practice standards
• Local candidates accepted if permanent residence is within 60 miles of Boston, MA, with rate adjusted accordingly
• Start date is contingent upon credentialing clearance and may be rescheduled if not cleared by the posted start date
