American Traveler is hiring a travel RN Home Health Case Manager in the Oshkosh/Fox Valley area, WI, with at least 1 year of RN experience and a valid driver's license required.

Details

• Home health setting serving patients throughout the Fox Valley and Oshkosh, WI areas
• Caseload averages approximately 165 census with 6 visits per day
• Common patient diagnoses and care types include wound care, COPD, CHF, diabetes, total joints, comprehensive assessments, medication management, IV medications, and PICC line management
• Monday through Friday, 8:00 AM – 4:30 PM, 8-hour day shifts, 40 hours per week
• Weekend backup rotation shared with other nurses, approximately 1–5 weekends per assignment
• On-call required one night per week and alternating weekends
• No holiday coverage required
• No floating required
• EMR: Epic Remote Client and Rover (experience a plus)
• IV pump: Curlin; equipment includes a laptop, mobile phone with hotspot, home care bag, and medical supplies

Requirements

• Active WI or compact RN license required
• BLS certification (AHA) required — must be obtained within 6 months of start if not already held
• Valid driver's license required — a personal vehicle is required for this position
• Associate's Degree in Nursing (ADN) or higher
• Minimum 1 year of RN experience required; 2 years of home health RN experience preferred
• Most recent experience must be within the last year
• Epic EMR experience preferred
• Professional reference from a Supervisor, Manager, Director, or Charge RN dated within the last year required for consideration
• Candidate DOB and last 4 digits of SSN, as well as middle name or initial, required for consideration
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months to be eligible

Additional Information

• Manages an individual patient caseload including initial assessments, plan of care development, direct patient care, and ongoing evaluation of patient response to treatment
• Completes clinical documentation including visit assessments, OASIS, 485 forms, and verbal orders in accordance with agency, state, and federal standards
• Communicates regularly with physicians and the broader care team to confirm and update patient plans of care
• Facilitates discharge planning to ensure continuity of care post-discharge
• Participates in chart audits, quality improvement initiatives, and process improvement activities
• Supported by a multidisciplinary team including PT, PTA, OT, LPN, MSW, and RN colleagues
• Orientation includes a new employee orientation (NEO) plus approximately one week of unit orientation
• No specific scrub color requirement
