American Traveler is seeking an experienced RN Case Manager for a 13-week inpatient care management assignment at a short-term acute care hospital in Portland, OR, requiring a minimum of 3 years of inpatient care management experience and active Epic proficiency.

Details

• Short-term acute care hospital setting with 418 staffed beds
• Inpatient Case Management/Care Management unit
• Patient caseload ratio of 20:1
• Day shift schedule, 5x8-hour shifts (8:00–16:30)
• Every other weekend required
• Floating between units and facilities across the health system is expected and required based on operational needs; refusal to float may result in assignment termination
• Facilities within the float pool include locations in Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent
• No on-call required
• Variable/balanced scheduling expected to support coverage during summer months
• Charting in Epic (experience required)
• Scrubs: any color permitted

Requirements

• Active OR RN license required
• Minimum 2 years of RN experience required; minimum 3 years of inpatient care management experience specifically required
• Recent acute care hospital experience in care management or discharge planning required; behavioral health experience does not satisfy this requirement
• Epic EMR proficiency required
• A personal vehicle is required for this assignment
• At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting required for consideration
• SSN and DOB required for consideration
• Active professional license at time of consideration
• All required certifications must be current and available for consideration

Additional Information

• Responsibilities include inpatient care management and discharge planning across units in a large acute care health system
• Travelers may float to multiple facilities within Oregon based on staffing needs
• An additional $5/hr incentive plus IRS mileage reimbursement is provided for hours worked at the Seaside facility when floating from a non-Seaside assignment; hotel accommodations are offered for consecutive floating shifts to Seaside
• Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility
• Minimal RTO is expected for this position; pre-approved RTO must be disclosed at the time of submission, as post-offer requests may be denied
