American Traveler is hiring an experienced RN Hospice Case Manager for a home hospice position in the Southeast Suburbs of Illinois, requiring a minimum of 3 years of home hospice or home health experience and an active IL RN license.

Details

• Home hospice setting — travel to patients' homes within a coverage territory including Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, and Dolton
• Adult patient population with diagnoses including failure to thrive, advanced cancers, end-stage COPD, and other end-stage/terminal disease processes
• Typical caseload of 12–15 patients
• Specialized skills include wound care, dressing changes, wound VACs (photographed and measured weekly), drain management (Pleurex, nephrostomy, foley), pain/symptom management, triage, and end-of-life care planning
• EMR: Epic Remote Client (experience preferred)
• Monday–Friday schedule, 8-hour day shifts, 0800–1630
• No weekend rotation, no on-call, and no holiday requirements
• 13-week contract

Requirements

• Active IL RN license required
• Current BLS certification required
• Valid driver's license, personal vehicle, and auto insurance required
• Minimum 3 years of home hospice or home health RN experience
• Most recent experience must be within the last year
• Strong organizational and time management skills essential
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the past year required for consideration
• Candidate DOB and last 4 digits of SSN required for consideration

Additional Information

• Responsibilities include care coordination, patient and family education, end-of-life planning, and ongoing pain and symptom management
• Quarterly in-person team meetings held at Trinity Hospital; initial supply pickup at the regional office in Oak Brook
• Traveler will be provided a phone, laptop, and trunk stock
• Support available from other hospice RNs and management via phone
• Orientation is 1–2 weeks based on comfort level and experience
• Scrubs must be navy blue or a solid professional color — no graphics, logos, or designs
