American Traveler is seeking a licensed Physical Therapist with home health experience for a full-time, 13-week contract covering Van Buren, Calhoun, and Kalamazoo County in Michigan.

Details

• Home health setting covering Van Buren, Calhoun, and Kalamazoo County
• Full-time schedule, Monday through Friday, 8:00 AM - 5:00 PM (5x8-hour shifts)
• Every 4th weekend required
• No call or float requirements
• 13-week contract

Requirements

• Active Michigan PT license required (license required if applicable)
• Current BLS certification (AHA) required
• Minimum of 1 year of home health experience required
• Valid Michigan driver's license, daily access to a personal vehicle for business use, and proof of auto insurance required

Additional Information

• Responsibilities include evaluating and treating patients with injuries, musculoskeletal, neurological, and bone diseases to restore function, improve strength, relieve pain, and enhance independence
• Clinical duties include functional assessments, treatment planning, patient and family education, goal setting, and direct individual or group PT interventions
• May design and fabricate splints and other assistive devices as needed
• Participates in interdisciplinary team discussions, program development, and department process improvement
• Documentation responsibilities include timely and accurate entries in the electronic health record per compliance timelines
• A phone interview with the team leader will be conducted prior to placement
