American Traveler is seeking an experienced RN for a Clinical Care Coordinator (Case Management) position in an acute care inpatient setting, requiring prior care coordination experience within a CommonSpirit/Dignity/CHI facility.

Details

• Inpatient acute care setting focused on discharge planning and care coordination
• Caseloads typically range from 15–28 patients per day
• Patient population includes all payer types, uninsured, and homeless individuals
• Day shift, 8-hour shifts
• No on-call requirements
• Charting performed in Cerner EMR

Requirements

• Active CA RN license required (Compact/NLC accepted); must be active at time of consideration — pending licenses not accepted
• Current BLS certification required; pending certifications not accepted
• Minimum 3 years of nursing experience in an acute hospital setting
• Prior care coordination or house supervisor experience specifically within a CommonSpirit, Dignity, or CHI facility is required
• Proficiency with Cerner EMR
• Strong interpersonal, written communication, and customer service skills
• Ability to multitask, work independently, and thrive in a fast-paced environment with shifting priorities
• A valid driver's license is required for consideration
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire

Additional Information

• Responsibilities include completing initial assessments, discharge planning (nursing home placement, home health, DME, prior authorizations, etc.), and managing care across all payer types
• Business casual attire with a lab coat required
• Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity facility within the past year are not eligible; candidates placed PRN through outside staffing firms will be considered
• 12-hour orientation included
